If ApoB has appeared on your results and you're not sure what it is, here's the short version: it counts the particles in your blood that can drive the process behind heart disease — and many people think it's a more informative number than the standard cholesterol figures they've watched for years. It's just less familiar, because standard panels have led with LDL for decades.

What ApoB actually measures

Apolipoprotein B (ApoB) is a protein that sits on the surface of the cholesterol-carrying particles most associated with clogging arteries — LDL and a few of its relatives. Crucially, there's one ApoB protein per particle. So measuring ApoB is effectively counting those particles.

That's the key idea. Standard cholesterol tests measure the amount of cholesterol being carried. ApoB measures how many particles are carrying it. It's the difference between weighing the total cargo and counting the lorries.

ApoB vs LDL: why the distinction matters

LDL-C — the "bad cholesterol" on your report — measures the cholesterol inside your LDL particles. Most of the time, LDL-C and ApoB agree, and either tells a similar story.

The interesting cases are when they disagree. Two people can have identical LDL-C but very different particle counts — one carrying a lot of cholesterol in a few large particles, the other spreading it across many small ones. Since it's the particles themselves that interact with the artery wall, the person with more particles may carry more risk even though their LDL-C looks the same. That's why, when the two numbers diverge, many clinicians treat ApoB as the more reliable signal.

Why isn't it on every panel?

Mostly history and cost. LDL-C is cheap, long-established, and baked into standard testing, so it remains the default. ApoB is a little more expensive and has only more recently moved into mainstream discussion — which is why it can feel "new" even though the underlying biology isn't. If it's not on your panel, it's a reasonable one to ask your GP about.

Optimal versus "in range"

Here's the part standard reporting tends to miss: ApoB often isn't flagged until it's fairly high, but the level associated with the lowest long-term risk sits well below that flag. So an ApoB result can be technically "in range" and still be some distance from where someone focused on long-term cardiovascular health would want it. Where your number sits within the range is more informative than whether it tripped a flag — but what any specific value means for you depends on your whole picture, which is a conversation for your GP or a lipid specialist.
