What we look at
The NHS uses population reference ranges — built to catch disease, not optimise health. Vigil maps every marker against longevity-specific thresholds derived from the peer-reviewed literature. The difference is often significant. And it's almost never flagged.
ApoB
Heart & circulation
The particle that actually deposits cholesterol into artery walls. A more accurate signal of cardiovascular risk than LDL — yet your GP is unlikely to order it unless you push. Longevity ranges are considerably tighter than clinical normal.
Lp(a)
Heart & circulation
Genetically determined and largely unchangeable — which makes knowing it early all the more important. Elevated Lp(a) is the most common inherited cardiovascular risk factor, present in roughly 1 in 5 people. Almost never tested routinely.
HbA1c
Blood sugar
Your three-month average blood sugar. Even sub-diabetic elevations compress healthspan — accelerating cellular ageing and increasing risk across multiple systems. Most GPs won't act until the reading approaches diabetic range. Vigil flags it sooner.
Fasting glucose
Blood sugar
A snapshot of insulin sensitivity. Read alongside HbA1c, it reveals whether blood sugar dysregulation is recent or chronic. The optimal longevity range sits below what most labs would mark as abnormal.
hs-CRP
Inflammation
Low-grade chronic inflammation is the common thread behind most age-related disease. hs-CRP is its most sensitive early signal — detectable years before symptoms emerge. Standard CRP tests miss the range that matters for longevity.
Testosterone
Hormones
Relevant for both men and women, though at very different ranges. Suboptimal levels affect energy, body composition, mood, and cognitive sharpness years before a GP would intervene — because GPs act on clinical deficiency, not optimal function.
SHBG
Hormones
Sex hormone-binding globulin determines how much of your testosterone is actually available to tissues. Total testosterone can look fine while free testosterone is low — SHBG is why. Rarely interpreted in context on standard results.
Ferritin
Inflammation & nutrients
Both a marker of iron stores and an acute-phase protein — meaning it rises with inflammation. The optimal range is a narrow window: too low signals depletion, too high signals either iron overload or systemic inflammation. Standard ranges miss both ends.
